Improving Population Health
The NYS Prevention Agenda 2013-2018 is New York State’s innovative blueprint for mobilizing public and private sector resources toward improving population health and reducing health disparities.
The Advancing Prevention Project (APP), a project of the New York Academy of Medicine, supports local-level implementation of the Prevention Agenda with backing from New York State Health Foundation.
The project is building the capacity of local health departments (LHDs) and hospitals to implement actionable Prevention Agenda plans.
The Advancing Prevention Project provides opportunities for collaboration and technical assistance to local health departments to support the implementation of the Chronic Disease and Mental Health/Substance Abuse priority areas of the Prevention Agenda.
APP offers the following services to local health departments:

• Learning Collaboratives
Last year, nearly 300 suicide prevention stakeholders joined together in Albany, NY for the first Annual New York State Suicide Prevention Conference.
