Improving Population Health
The NYS Prevention Agenda 2013-2018 is New York State’s innovative blueprint for mobilizing public and private sector resources toward improving population health and reducing health disparities.
The Advancing Prevention Project (APP), a project of the New York Academy of Medicine, supports local-level implementation of the Prevention Agenda with backing from New York State Health Foundation.
The project is building the capacity of local health departments (LHDs) and hospitals to implement actionable Prevention Agenda plans.
The Advancing Prevention Project provides opportunities for collaboration and technical assistance to local health departments to support the implementation of the Chronic Disease and Mental Health/Substance Abuse priority areas of the Prevention Agenda.

Upcoming Learning Collaborative: Promoting Complete Streets
This 3-part learning collaborative will offer participants peer-to-peer learning opportunities and technical support on how to implement Complete Streets policies at the county and municipal levels in New York State.
Through a mix of virtual meetings and one-on-one technical assistance sessions, participants will learn how to implement the Complete Streets components of their Community Health Improvement Plans. To learn more, download the Complete Streets Learning Collaborative flyer.
